Drug treatment (oral therapy)

The first step in treatment is often a PDE5 inhibitor. This medication improves blood flow to the erectile tissues of the penis, but it only works with sexual stimulation. There are four active ingredients available, each with its own characteristics. Sildenafil lasts 4 to 6 hours and is taken about an hour before sexual activity—the classic on-demand treatment. Tadalafil lasts up to 36 hours and can be taken either on-demand or daily in a low dose, allowing for greater spontaneity. It also has a beneficial effect on urinary symptoms caused by an enlarged prostate. Vardenafil lasts 4 to 8 hours and may be more effective in certain metabolic profiles. Avanafil is the most selective compound, with a rapid onset of action (15 to 30 minutes) and potentially fewer side effects. The choice of compound, dose, and timing of administration is tailored to your cardiovascular profile and lifestyle.

Local drug therapy

When oral therapy is ineffective or cannot be taken, alprostadil is a reliable alternative. This is a prostaglandin that locally dilates the blood vessels in the penis and works without the need for sexual stimulation. It is administered through the urethra as a gel or cream. The effect sets in after 5 to 30 minutes. This treatment is suitable for patients who cannot take PDE5 inhibitors, for example, due to a cardiovascular condition or interactions with other medications.

Herbal Medicine and Libido

Ashwagandha (Withania somnifera) is an adaptogenic herb used in Ayurvedic medicine that is increasingly being studied in the context of male sexual health. Potential effects include stress reduction through lowered cortisol levels, support for testosterone levels in certain patients, and improvements in libido and overall vitality. Herbal medicine is not a substitute for medical treatment when the cause is vascular or neurological. Supplements vary widely in quality and dosage, and medical advice is always recommended.

Mechanical aids

Mechanical aids can be very effective, but not every device on the market is certified for quality or safety. A vacuum pump creates negative pressure, drawing blood into the penis. It is often used in combination with a penis ring that traps the blood and maintains the erection. A penis ring or constriction ring can also be used on its own—in this case, it is important to consider the correct size and the maximum wear time. In cases of structural instability, supportive sleeves or stabilizing devices may offer a solution. Injection Therapy and Regenerative Treatments

When oral medication is not effective enough, there are powerful and often highly effective alternatives available. PRP, or Platelet-Rich Plasma, is a regenerative treatment that uses the patient’s own blood. After collection and centrifugation, the platelet-rich plasma is injected into the corpora cavernosa. This stimulates the formation of new blood vessels, promotes tissue regeneration, and supports the natural erectile response. PRP is particularly useful for vascular erectile dysfunction, early endothelial damage, and selected post-prostatectomy patients. BOCOX is a treatment involving very low-dose botulinum toxin injected into the corpora cavernosa. It relaxes smooth muscle tissue, reduces excessive sympathetic activity, and improves the vascular response. This can be beneficial for patients with an inadequate response to PDE5 inhibitors or for those with functional erectile dysfunction characterized by increased adrenergic tone. The treatment is minimally invasive and is performed under medical supervision. Shockwave, or LI-ESWT, stands for Low-Intensity Extracorporeal Shockwave Therapy. This non-invasive treatment stimulates microvascular regeneration, improves endothelial function, and promotes the formation of new blood vessels. It is administered over multiple sessions and is particularly effective for mild to moderate vascular erectile dysfunction.
