Recent studies show a significant decline in sperm concentration: more than 50% between 1973 and 2018. Sperm quality is also deteriorating. This is linked to environmental factors such as exposure to endocrine disruptors, lifestyle factors (smoking, stress), and the rise in obesity. The Importance of an Accurate Diagnosis
An accurate diagnosis is the first essential step toward understanding the causes of subfertility and determining the appropriate treatment. This requires several tests:
Semen analysis: assesses the quality and quantity of sperm (volume, concentration, motility, and morphology). This is often the first test, as it provides important information about fertility.
Blood tests: a hormone test (to measure the levels that regulate sperm production), an infection test, and a genetic test for chromosomal or genetic abnormalities.
Scrotal ultrasound: examines the testicles and surrounding structures for abnormalities such as a varicocele (dilated veins).
Transrectal ultrasound of the prostate: evaluates the prostate and seminal vesicles and detects obstructions or abnormalities that may impair ejaculation or sperm production.
MRI of the prostate or pelvis: to obtain a clearer picture of the pelvic structures, especially when obstruction or complex abnormalities are suspected.
Together, these tests provide a comprehensive picture of reproductive health and identify the exact cause of subfertility.

Available Treatments
Once a diagnosis has been made, there are several treatment options available, depending on the identified cause:
Medication: to improve sperm quality and production, especially in cases of hormonal imbalance or infection.
Micro-TESE (testicular sperm extraction): a surgical procedure to extract sperm directly from the testicles, even in cases of azoospermia (absence of spermatozoa in the semen).
