Penile prostheses have been around for more than 50 years and are an established, reliable treatment for men with severe and persistent erectile dysfunction who do not respond to medication or other therapies.
This surgical procedure involves placing a prosthesis in the penis, making it mechanically possible to achieve an erection. The goal is to allow men to once again enjoy a satisfying sex life when other treatments have been ineffective.

A lasting solution for severe erectile dysfunction
An erectile prosthesis is usually considered when:
- Medication (such as PDE5 inhibitors) does not work or is not tolerated
- Injections or vacuum pumps do not offer a solution
- Erectile dysfunction can result from medical conditions such as diabetes, cardiovascular disease, nerve damage, or trauma.
- There is permanent loss of erection after, for example, a prostatectomy.
The treatment is scientifically proven and is used worldwide in specialized urology centers.
Types of penile implants
1. Hydraulic (inflatable) penile prosthesis
This is the most common type and best mimics the natural mechanism of an erection.
- The prosthesis consists of two cylinders in the penis, a pump in the scrotum, and a reservoir containing sterile fluid that is placed under the abdominal wall.
- By pressing the pump in the scrotum, fluid is pumped into the cylinders, creating an erection suitable for sex.
- After sexual activity, the fluid is released again through a smaller valve, causing the penis to become flaccid.
- This prosthesis most closely resembles a natural erection in terms of feel and appearance; the penis is not conspicuous when flaccid.
- Operating the pump requires some hand coordination, which may be a factor for some patients.
Hydraulic systems are sometimes further subdivided into two-part or three-part systems, but the essence remains that they operate with an internal pump and fluid reservoir to provide flexibility and control.
2. Flexible (semi-rigid) penile prosthesis
This is a simpler system consisting of two flexible rods that are permanently placed in the penis.
- The penis always remains in a semi-erect state.
- For sex, the penis is manually bent upward and then brought back down for rest and comfort.
- The advantage of this type is its simplicity: there are no pump systems or reservoirs, which reduces the risk of mechanical problems.
- It may be a good option for patients with reduced dexterity or who want a less complex implant.
Although this prosthesis feels less natural than a hydraulic version, it remains a reliable and user-friendly solution for many men.
Important to know
- Both types are completely implanted in the body and are not visible from the outside.
- After implantation, spontaneous erections can no longer occur because the natural erectile tissue is replaced by prosthetic materials, but the control system allows erections on demand.
- The choice between the two types depends on medical factors, daily hand functions, expectations, and patient preference. This is always discussed between the patient and urologist.
Who are implants intended for?
An erectile prosthesis is usually considered for men:
- Those who do not achieve sufficient results with medication or other therapies
- After radical prostatectomy with permanent erectile dysfunction
- With severe vascular or neurological damage
- With long-term diabetes and severe erectile dysfunction
It is important to emphasize that this is a carefully considered decision. An erectile prosthesis is not used as a first-line treatment, but can be a highly effective and long-lasting solution for the right patient.
